Paw Service Forms
Once a Paw Caregiver has reviewed your Service Request, you will be contacted within 48 hours.
A Paw Caregiver will meet you and your pet(s) before your service date. Once your introduction visit is scheduled, the other 3 forms MUST be submitted.

Yes, it is a little time consuming but they will only need to be submitted once unless anything changes in the future like adding a new pet or changes to address, vet information, etc.

For insurance purposes, We CANNOT provide your service without these forms.
In the event of an emergency, your Paw Caregiver will make every attempt to contact you, your secondary pet parent emergency contact, and listed Veterinarian.
In the event that no contact can be reached, your Paw Sitter will seek appropriate medical care for your pet(s).

If your pets do not go to the same vet clinic, you will need to fill out a new form for each pet and clinic.
This agreement MUST be submitted before Pawsitive Dog LLC will provide you and your pets for any service.

Payment in full is due prior to service unless special arrangements are agreed upon by both parties in writing.

Unpaid service may be cancelled without notice, including prior to or during the service period.
